Polk Wellness Center is a 501(c)(3) not-for-profit organization. In order to fulfill our Mission/Vision and meet the growing need for services in our area, we need your support. There are many ways to contribute to our cause.

Polk Wellness Center formed in 2008 as a local solution to a national problem: the lack of affordable medical and mental healthcare set against the backdrop of an increasing need for services. After convening community leaders and clinicians to study the problem and put in place a solution-based plan, Polk Wellness Center opened its doors to treat patients and clients with insurance and without. Using an integrated care model, Polk Wellness Center offers a forward-thinking approach to treatment—empowering patients/clients to take control over their health and wellness.
